New Posting Case Manager
Provide support to assigned clients. Facilitate / link clients to appropriate community service resources. Assist and support clinical services as needed in cooperation with the treatment team.

You will provide the necessary skill training and one on one supervision to clients as outlined in their treatment plan. Your immediate staff family will include a program coordinator, primary therapist or on-call staff as needed. Maintain case records and documentation in accordance with PWC policies and procedures. The Peak Team Intensive Care Coordinator will provide on-going assessment, crisis intervention, individual, family, and group therapy and case management services. Develops strength-based service plans, monitors treatment progress, documents interactions and maintains electronic clinical records and other reporting data for individuals with mild to severe mental illness and/or substance use disorders. Coordinates with internal as well as external supportive services to plan and arrange for necessary services to meet the client's needs post hospitalization. Ensures professional interface with Behavioral Health Organization partner for utilization management. Collaborates with clinical supervisor, other team members and psychiatrist regarding treatment.
